What is CHADIS?
CHADIS (Child Health and Development Interactive System) is an interactive online screening tool designed to provide your children with the best care possible. CHADIS does this by assisting in the early diagnosis of developmental, social, emotional, and behavioral concerns as well as a variety of health risks that can be difficult to see during a typical visit. All responses are completely confidential and cannot be viewed by anyone except your doctor.
HOW TO USE CHADIS
Conveniently provide important information your doctor needs by completing questionnaires that evaluate health, behavior, or development.
- You will receive email or text requests to login to your patient portal and complete the questionnaires in CHADIS.
- Please click the link in the email or text to log in to your practice’s patient portal.
- Click the orange “Start Questionnaires” button to complete the questionnaires before your appointment.
What can CHADIS do for you?
Using your smartphone, tablet, or computer, with CHADIS you can:
- List Your Concerns
When you fill out your CHADIS questionnaires, this is a time you can communicate to your health team any health concerns and define the purpose of your visit. - Provide Health History
The office may ask you to give or update your child’s health history or tell them about any changes in your health since your last visit. - Fill Out Health-Related Questionnaires
The CHADIS questionnaires may ask you questions about your physical or emotional health and will help your health care provider plan how to direct your visit and provide you with the highest level of health care. If this is a visit for your child, the questionnaires will also help show how your child is developing for their age. Teens have their own private access to fill out their questionnaire and allows them to be more open about their concerns. - Get Input From Teachers or Caregivers
Teachers and caregivers can fill out questionnaires for your child which will provide a larger picture of your child’s health and behavior needs and increase their ability to succeed. - Access Educational Materials
Capture your child’s special memories and achievements in a keepsake format, find suggested educational activities based on your child’s development, access necessary educational paperwork, access local or national resources and other interesting educational materials.
Telemedicine Visits with Doxy.me
Our telemedicine platform Doxy.me offers patients and their parents a new means to connect with our health providers. Telemedicine visits will be done with live audio-video technology in a HIPPA-compliant fashion.
While not every complaint will be suitable for a virtual visit, our providers will work as appropriate through this platform to review your concerns, answer questions, review medications, order testing, and coordinate referrals.
Please call our office to make telemedicine appointments. You do not need a Doxy.me account. The visit can be done from a computer or mobile device.
At the time of your appointment, you will receive an e-mail with a direct link. Please be sure you are on a WiFi with a strong signal in a private location. Your child must be present for most virtual visits, unless it is a parent to provider consultation visit.
Please Note
- We reserve the right to request a patient come into the office to be seen.
- Co-pays will apply to telemedicine visits and can be paid on the patient portal prior to your visit.
- Policies are subject to change.
Tips for Your Telemedicine Visit
- Good lighting is helpful. Place the light in front of you and your child, rather than behind you.
- Please have your child’s recent weight and temperature.
- We may need to review any medications your child is using, and verify your preferred pharmacy.
Privacy Considerations
Please understand that while this software is HIPAA-compliant, by choosing to do a virtual visit, you are agreeing to electronic communication of you or your child’s health information through audio, video, and/or photo technology. While our software is secure, consider any privacy needs in your space as well.
